Waiver

Acknowledgment and Assumption of Risk

I, the undersigned, acknowledge that participation in martial arts training at Hybrid Training Windsor involves inherent risks, including but not limited to physical injury, disability, and, in rare cases, death. I understand that martial arts is a contact activity and that injuries may result from my participation, including those caused by falls, contact with other participants, equipment failure, or the conditions of the facility.I voluntarily assume all risks associated with my participation in martial arts training and acknowledge that Hybrid Training Windsor, its instructors, staff, affiliates, and representatives are not responsible for any injuries, damages, or losses that I may sustain.

Release of Liability

I hereby release, discharge, and agree not to sue Hybrid Training Windsor, its owners, instructors, employees, agents, or representatives from any and all claims, liabilities, demands, or causes of action that may arise from my participation in activities at the school, whether caused by negligence or otherwise. I understand that this release includes any claims based on the actions, omissions, or negligence of Hybrid Training Windsor, its employees, volunteers, and representatives.

Medical Authorization

In the event of an emergency, I authorize Hybrid Training Windsor and its representatives to seek medical attention on my behalf if I am unable to do so. I understand that I am responsible for any medical expenses incurred as a result of my participation in martial arts training.

Rules and Code of Conduct

I agree to abide by all rules and regulations set forth by Hybrid Training Windsor. I acknowledge that failure to follow instructions or engage in unsafe behavior may result in my dismissal from the program without refund.

Indemnification

I agree to indemnify and hold harmless Hybrid Training Windsor, its instructors, employees, and affiliates from any claims, damages, or costs incurred due to my participation in martial arts activities.

Photo and Video Release

I grant permission for Hybrid Training Windsor to use photographs or videos of me for promotional, instructional, or advertising purposes without compensation. If I do not wish to be photographed or recorded, I will notify the school in writing.

Acknowledgment of Terms and Conditions

I have read and understood the Terms and Conditions and Privacy Policy of Hybrid Training Windsor and agree to be bound by them.
